David M. Ochs, MPT, Clinic Director
David has a master's degree in Physical Therapy from Nova
Southeastern University, and has been practicing outpatient
orthopedics since 1996 in Gainesville and Ocala.
He received his undergraduate degree at the University of
Delaware in Exercise Science and was a scholarship athlete
in football. Much of his post education has been focused
on manual treatment, aquatics, functional capacity
evaluations and physical reconditioning evaluation and
treatment. |

Charlene M. Frazer, PT
Charlene received her bachelor's degree in Physical Therapy
from the University of Florida in 1989. She has worked in
a number of private outpatient clinics both in Florida and
California. Her clinical interest is in orthopedics. Charlene
specializes in manual therapy techniques and has extensive
experience working with the foot and ankle. |

Brittany Cole, MOT,
OTR/L, ATRIC
Brittany
received her Master of Occupational Therapy from the University of Florida
in 2005. She has
worked in outpatient orthopedic hand therapy for the past
four years in Washington
and Arizona and relocated
back to Florida in 2009.
Areas of interest include custom splinting, manual
therapy, congenital hand anomalies, activity analysis and
modification, and pediatric aquatic therapy.

Jessica Whitely, PT
Jessica received her Bachelors degree in
Rehabilitative studies from Stetson University in 2006 and
her doctorate in Physical Therapy from the University of St.
Augustine in 2010. Her area of interest is in orthopedics,
manual therapy and sports related injuries.

Ludo De Wolf, PT, Clinic Manager
Ludo received his bachelor's degree in Physical Therapy from
the University of Leuven in Belgium in 1988. He relocated
to Florida in 1989 working in neuro rehabilitation. In
2001
he received his master's degree from the University of Florida
in Health Science and since then he has been focusing
more on patients with orthopedic problems. |

Mark F.
Juario, PT
Mark received his Bachelor of Science in
Physical Therapy from Cebu Doctors' University in the
Philippines in 1999. During his clinical internship, he was
exposed to different clinical settings such as geriatric,
pediatrics, acute and long term care, cardiac and sports
rehabilitation. He relocated to Florida in March 2007 and
joined Fit for Life last December 2007. His areas of
interest are in orthopedics and aquatics. |

Genné
McDonald, PT
Genné
is a 1989 graduate of Oakland University and since 1989 has
spent treating women's health,
outpatient orthopedic and oncologic conditions. She has
successfully developed and been the lead clinician for
three women's health programs in Colorado Springs,
Colorado and in Gainesville Florida. Most recently she
was the lead clinician in developing the Rehab Services
at the Davis Cancer Center and as a clinical faculty
member in the University of Florida Department of
Physical Therapy. She has presented at national nursing
and physical therapy conferences on the treatment of
pelvic pain, urinary incontinence, and postural
evaluation of the pregnant patient, and written articles
on rehabilitation for the breast cancer patient and
pregnant patients with musculoskeletal pain. She has
won numerous state and national awards for her volunteer
work on behalf of cancer survivors.

Nicholas Machupa, PT,
OCS
Nick is a graduate of Columbia University
in 1977 with most of his experience
focused on outpatient orthopedics with
a emphasis in unusual pain patterns in spinal rehabilitation,
both cervical and lumbar. Nick has training in joint and soft
tissue mobilization and use of proprioceptive neuromuscular
facilitation related to orthopedic problems. Board-certified
by the American Physical Therapy since 1990 in orthopedics. |

Michael Alvarez, PTA
Michael received his bachelor's degree from the
University of Central Florida in 2006 and his Physical
Therapist Assistant degree in 2009 from St. Petersburg
College. During his clinical experience, he was exposed to
geriatrics, neurological disorders, acute care and
outpatient care. His area of interest is in orthopedics,
manual therapy and aquatic therapy.

Ginny McLeod, PTA
Ginny has extensive experience as a licensed physical therapy assistant, receiving her degree from St. Petersburg College in 1977. She has worked in a variety of settings including, acute and sub-acute hospital care, home health, and orthopedic outpatient facilities. Her areas of interest and experience include manual therapy,
aquatics as well as body mechanics, posturing, positioning, and strength building.

Cindy Miller,
PTA
Cindy is a graduate of Herkimer County Community College in Upstate New York. She is a licensed Physical Therapist Assistant that has worked primarily in outpatient orthopedic settings. Her experience ranges from orthopedic sports related injuries to Aquatic and gait/balance disorders. |

Megan C. Surufka, DPT
Megan received her Doctorate of Physical
Therapy from Northwestern University after receiving her
degree in Psychology from University of Illinois at Chicago.
She presented her research discussing student’s
perspectives of hospital-based physical therapy at the 2010
Combined Sections Meeting in San Diego, CA and has focused
her treatments mainly on orthopedics, vertigo, balance
disorders, and aquatic therapy.

Keith received his Bachelor of Science in
Physical Therapy from Cebu Doctors' University in the
Philippines in 2000. He relocated in Florida in March 2007
and joined the company in December 2007. His clinical area
of interest is in orthopedics. |

DeWayne Purvis, PT
DeWayne received his Bachelors degree in Exercise and Sports Sciences, specializing in Athletic
Training, from the University of Florida in 2006 and his Doctorate in Physical Therapy
from the University of St. Augustine for Health Sciences in 2010. His area of interest
is orthopedics, manual therapy, and sports physical therapy. |

Lynn C. Jones, OTR/L
Lynn completed her Bachelor of Science Degree at Florida International University in 1977. Initial interests were in-patient rehabilitation in both Jacksonville and then Miami. While in Miami, Lynn developed an interest in hospital based practice and orthopedics. She moved to Southern Virginia pursuing hospital based in-patient, out-patient and pediatrics therapy practice. Upon her return to Florida in 1995 she settled in Lake County and in 1996 began working in a private out-patient practice where she has pursued her interest in orthopedics and hand therapy for the last 13 years. Her primary interests are work related injuries; activity analysis and accommodation; and bum injuries. |
